The Effect of an Indirect Anchorage Reinforcement Device on Maxillary Anterior Intrusion with Clear Aligners: A Three-Dimensional Finite Element Analysis and Exploratory Clinical Validation

Achieving predictable maxillary anterior intrusion remains one of the most formidable biomechanical challenges in contemporary clear aligner therapy.

While deep overbite correction and anterior overbite control during space closure are routinely incorporated into digital treatment plans, clinical expression often falls short of target movements.


A primary constraint lies in the side-effect dynamics of clear aligners: compressive intrusion vectors applied to the anterior segment frequently trigger reactive posterior extrusion and unwanted tipping, leading to a loss of vertical anchorage and compromised treatment efficiency.

To address these biomechanical limitations, this landmark two-part study introduces and validates an innovative Indirect Anchorage Reinforcement (IAR) device designed to guide occlusal contact and reinforce posterior vertical stability during active intrusion.

Combining high-resolution 3D Finite Element Analysis (FEA) with an exploratory clinical study, the authors systematically evaluated the initial force-moment systems, tooth displacement patterns, aligner stress distribution, and actual clinical movement accuracy under both non-extraction and extraction mechanics.


The findings demonstrate that incorporating the IAR device significantly suppresses parasitic posterior extrusion while optimizing initial force distribution, allowing the resultant force to act much closer to the center of resistance of the anterior teeth.

Clinically, this translates into a substantial increase in true intrusion accuracy—yielding a net gain of nearly 19 percentage points for central incisors and over 33 percentage points for canines.

By establishing a robust framework for vertical anchorage control, this research offers clinicians a highly effective, evidence-based auxiliary protocol to enhance deep bite predictability and elevate outcomes in clear aligner orthodontics.
