The internal anatomy of the Maxillary first premolar is notoriously variable — a fact that can complicate endodontic and restorative procedures.

In the case presented by Jethi et al., a single-rooted premolar with two separate canals formed a classic “bayonet” (S-shaped) radicular configuration.


The double curvature significantly increases the risk of iatrogenic complications such as instrument separation, ledges, or canal blockage during canal negotiation, instrumentation, and obturation.

This report describes a carefully executed post-endodontic reinforcement of such a premolar — before crown placement — using a novel technique aimed at preserving both structural integrity and functional longevity.
