PDF: 3D-Printing Assisted Micro-Osteoperforations: Accelerating Canine Retraction in Adult Orthodontics

Reducing overall treatment duration without inducing adverse biological sequelae remains one of the primary frontiers in contemporary adult orthodontics.

Managing malocclusion in adult patients frequently demands significantly prolonged treatment timelines compared to juvenile cohorts, largely due to diminished cellular vitality, reduced vascularity, and slower alveolar bone remodeling dynamics.

However, extended active mechanotherapy carries well-established clinical risks—including progressive enamel demineralization, marginal caries, periodontitis, and external apical root resorption.

To overcome these biological rate-limiting barriers, minimally invasive surgical adjuncts leveraging the Regional Acceleratory Phenomenon (RAP) have garnered substantial clinical interest, with Micro-osteoperforations (MOPs) emerging as a prominent flapless modality to trigger localized osteoclastic recruitment.

Nevertheless, executing conventional freehand micro-osteoperforations chairside poses distinct technical challenges, notably the risk of imprecise cortical penetration, inadvertent periodontal ligament violation, and localized root damage in narrow interradicular spaces.

To address these safety and accuracy limitations, the integration of digital 3D-printing technology offers a transformative leap: custom surgical guides ensure calibrated depth, angulation, and anatomical precision directly tailored to patient-specific CBCT and intraoral anatomy.

In this clinical investigation, the authors evaluate the efficacy and biomechanical outcomes of 3D-printing assisted MOPs specifically applied to accelerate canine retraction in demanding orthodontic scenarios.

By assessing retraction velocity and biological safety under guided cortical stimulation, this study delivers essential evidence-based insights for clinicians aiming to safely shorten treatment duration, safeguard periodontal architecture, and minimize iatrogenic complications in adult patients.
