Radiodiagnostics of Standard Orthodontic Radiographs: Dental and Extradental Incidental Findings

Standard diagnostic imaging—primarily orthopantomograms (panoramic radiographs) and lateral cephalograms—constitutes the cornerstone of comprehensive orthodontic assessment, treatment planning, and growth evaluation.

While these radiographic modalities are routinely prescribed to evaluate tooth presence, root morphology, eruption paths, and skeletal divergence, their field of view encompasses extensive craniofacial and cervical structures well beyond the dentoalveolar complex.


Consequently, clinicians are legally and ethically obligated to perform a thorough, systematic evaluation of the entire image to detect potential occult pathologies and anatomical anomalies.

However, in daily clinical practice, diagnostic attention is frequently concentrated exclusively on immediate orthodontic parameters, such as crowding, impactions, and sagittal discrepancies.

This localized focus creates a significant risk of overlooking subtle yet clinically relevant dental and extradental incidental findings—including paranasal sinus opacifications, temporomandibular joint degenerative remodeling, stylohyoid ligament calcifications, cervical spine vertebral anomalies, and benign or malignant bony lesions.

Because many of these extradental conditions remain entirely asymptomatic, standard orthodontic radiographs often serve as the primary opportunity for early detection and timely interdisciplinary referral.

This critical radiodiagnostic investigation evaluates the detection rate, diagnostic reliability, and distribution of incidental findings across routine orthodontic records.


The authors systematically examine the prevalence of abnormalities within nine distinct anatomical zones—spanning the dentition, maxillary and mandibular basal bones, paranasal sinuses, TMJ, cranial base, orbit, cervical spine, and adjacent soft tissues.

By analyzing the degree of inter-examiner agreement and the factors that influence diagnostic oversight, the study demonstrates that a substantial proportion of incidental findings located outside the dentition require further diagnostic workup and specialized medical or surgical intervention.
